It also has a proven capacity for local initiative: health homes, employee-run health centers, student reception, intercommunal cooperation, and mobilization to maintain essential services.

Yet access to healthcare is deteriorating in many local living areas.1. The lack of treating doctors and specialists, the difficulties in psychiatry, the overcrowding of emergency departments, the pressure on hospital and home care workers, aging populations and social inequalities create losses of opportunity. The administrative border separating the Loire-Atlantique from the other four departments fragments hospital planning, training, prevention and data at the scale of historical Brittany.

For Yes Breizh, health cannot be reduced to the consumption of care. It depends on housing, work, income, education, food, the environment, mobility, social ties, and the ability to take action over one's own life. Brittany must therefore shift the focus of repair from repair to prevention, from structures to pathways, from uniform standards to real equality for all of Brittany.

The devolution proposed by Yes Breizh would not mean the withdrawal of the Social Security system or the creation of a medicine system with multiple levels of service. The State and national solidarity would indeed continue to guarantee rights, benefits and equalization, but a Breton Health Authority2, A democratically controlled agency would receive the means to organize supply, prevention, training, environmental health, and cooperation across five departments.

The priorities: ensuring effective access to primary care in every living area;

protect a graduated, secure, and readable hospital network; ;

making mental health and suicide prevention a priority in Brittany; ;

investing in prevention, addictions and environmental health; ;

attracting, retaining and developing professionals according to territorial needs; ;

better coordinate the city, hospital, home, medical and social services and pharmacy; ;

involve patients, professionals and elected representatives in decision-making; ;

preserving financial solidarity and measuring quality rather than just activity.

Summary of the dossier

1. Indicators of success

1. A Breton conception of health

1.1 Health is a capacity to live

The World Health Organization defines health as a state of physical, mental, and social well-being, and not as the mere absence of disease. This definition requires action to be taken on the determinants of health and to consider care as a component of a broader policy.

For Yes Breizh, a good health policy is judged by concrete results: years of healthy life, access without loss of opportunity, reduction of social and territorial disparities, quality of life for patients, safety of care and working conditions for professionals.

1.2 Equality is not uniformity

For Yes Breizh, applying the same rule everywhere does not guarantee equal access. An island, a rural territory, a university metropolis and an industrial basin do not have the same needs or the same resources. Real equality requires equalization and differentiated models: liberal practice, salaried work, mobile teams, tele-health accompanied, health centers, advanced consultations, and cooperation between professions.

1.3 Trust as an infrastructure

The care relationship is based, according to Yes Breizh, on trust, confidentiality and consent. Public decisions must be transparent, but also clear (understandable), based on scientific knowledge and protected from conflicts of interest. Complementary approaches can only be supported when their effectiveness and safety are evaluated; they must never delay a diagnosis or a proven treatment.

2. Brittany faces a double demographic and health challenge

Indicator

Reference value

Perimeter and reading

Population as of January 1, 2026

5,032,600 inhabitants

Brittany – 5 or 7.28% % of the French population (including Brittany).

Share of those aged 65 and over

About 25 %

An aging rate higher than the national average

Life expectancy at birth in 2025

79.8 years for men; 85.6 years for women

Lower than the French average in the administrative region

Suicide deaths in 2023

975

Rate of 19/100,000 inhabitants in Brittany (+ 40 % / the French average)

Hospitalizations due to self-inflicted injuries in 2024

8 860

Standardized rate of 180 / 100,000 inhabitants (+ 27% compared to the French average)

Deficit of mandatory schemes and the FSV in 2025

21.6 billion euros

Data for France; deficit in the health sector: 15.9 billion. But in Brittany, healthcare is administered by two Regional Health Authorities (ARS), two regions, and several hospital groups from the ‘territoire’. The available statistics often relate either to the administrative region of Brittany or to the Pays de la Loire. The groupings are delicate.

2.1 An older and unevenly distributed population

As of January 1, 2026, a quarter of the inhabitants of the administrative region are at least 65 years old and the average age is approaching 44 years. Ageing increases the need for chronic care, home adaptation, coordination, and professionals for the elderly. It should not lead to a purely accounting approach: the goal is to prevent loss of autonomy and to allow people to choose their place of living.

2.2 Significant geographical disparities

Ille-et-Vilaine benefits from a younger population and a high concentration of training and specialties. Other parts of Brittany are experiencing aging, remoteness, low incomes, and the departure of professionals. For Yes Breizh, accessibility must be measured in real time: from obtaining an appointment, transportation, and care, not by the number of doctors per population alone.

2.3 A perfectible life expectancy

In 2025, regional life expectancy remains lower than the French average, for both women and men. The disparities between the departments are marked: Ille-et-Vilaine exceeds the national average, while the Côtes-d'Armor experiences the lowest levels in the administrative Brittany. This geography imposes reduction objectives for disparities and action on social determinants, addictions, mental health and access to care.

3. Access to healthcare: breaking away from the logic of isolated recruitment

3.1 The treating physician remains the pivot

For Yes Breizh, without a treating physician, monitoring chronic illnesses, prevention and guidance become more difficult. But attracting a practitioner through an advertisement or an empty building is not enough. Young professionals are looking for collective work, conditions compatible with their lives, access to housing, replacements, and the possibility of practicing their profession without administrative burdens.

3.2 The lessons of the Centre-Bretagne

The experience of Pays Centre Ouest Bretagne highlights three inseparable elements: a local health project, a multi-professional health center, and internship supervisors able to welcome students. Internships create concrete knowledge of the local area and can encourage the doctors setting up practice. The medical project must precede the real estate project.

In Maël-Carhaix in the Côtes d'Armor in Centre Bretagne, for example, the reception of interns and collective work have contributed to consolidating the offer. In Sourn in Morbihan, southwest of Pontivy, the employee health center launched in 2015 has been expanded through intercommunal cooperation and has contributed to the maintenance of pharmacies. These experiences do not constitute a single model, but they show the interest in giving the local area a stable capacity for experimentation.

3.3 Several statutes, the same requirement of quality

For Yes Breizh, the liberal practice, the municipal or intercommunal salaried employment, the mutualist centers and the hospital teams can coexist. The choice must be based on need and quality, not on ideological opposition. Initiatives involving retired doctors, such as in Plougasnou in the east of Finistère, can ensure a transition, provided that the requirements of competence, safety, continuity and workload are respected.

3.4 Telehealth as a complement

While teleconsultation can avoid certain travel and facilitate access to specialized advice, it does not replace clinical examination when necessary, human presence, or geographical organization. It must be accompanied by, accessible to, secure, and integrated into the patient's file.

3.5 A national signal to be read from the Breton departments (or countries)

The «health» wave of the Observatoire des territoires published in January 2026 indicates that 40 % French people find access to healthcare difficult where they live. This proportion reaches 53 % in rural communes and 52 % in agglomerations with fewer than 20,000 inhabitants. Moreover, 81 % declare having given up consulting at least once in the previous twelve months: 65 % cite excessively long waiting times and 60 cite % the absence of specialists nearby, up to 70 % in rural areas.

These data, obtained from a national study and not from an indicator specific to Brittany, are nevertheless a warning signal for the rural, island, and small town areas of this region. They call for measuring the real extent of abandonment in Brittany, its causes, and the disparities between different life zones, rather than relying solely on medical densities.

4. Hospital, emergency rooms and unscheduled care

4.1 For Yes Breizh, a network of graduated rather than an opposition between proximity and excellence

Not all hospitals can provide all specialties on a permanent basis. But any concentration must be compensated for by a safe organization: stabilized emergency departments, medical transports, advanced consultations, mobile teams, telemedicine, beds for emergency care, and transfer protocols. Closing a service without prior functional solutions creates a loss of opportunity and lasting distrust.

For Yes Breizh, the CHU of Rennes, Nantes and Brest should form a Breton alliance for rare care, research, training and strategic purchases. The general and nearby hospitals must be linked to this alliance according to clear responsibilities.

4.2 Emergencies reveal the failures upstream and downstream

According to Yes Breizh, the congestion is not only due to misuse by patients. It reflects the lack of available doctors, the inadequacy of unscheduled care, the difficulties of psychiatry, the lack of beds available for discharge, the fragility of the home environment, and transportation issues. The response must organize a complete chain rather than multiplying filters.

A Breton plan3 Hospital patient flow must follow the beds on a daily basis, delayed discharges, the needs for transport and the capabilities of the home and the social care team. Home hospitalization will be developed when it corresponds to the patient's choice, a safe medical indication and the effective presence of the necessary professionals and caregivers; it must never be used to transfer an uncompensated burden to families.

4.3 Measuring quality and results

Activity-based pricing has supported the measurement of activity, but can encourage volume and fragment the paths. The Court of Accounts highlights in 2026 the need to better control health insurance spending. This control must not be based on indiscriminate reductions: it must be based on the relevance of actions, prevention, coordination, reduction of avoidable complications, and the quality of results.

5. Mental health and suicide prevention

5.1 An explicit Breton priority

In 2023, 975 deaths by suicide were recorded across Brittany, representing a standardized rate of 19 per 100,000 inhabitants. In 2024, 8,860 hospitalizations for self-inflicted harm were recorded. Teenagers and young women have particularly high hospitalization rates, while men aged 45 and over experience the highest death rates.

All these results are higher than in the rest of France.

For Yes Breizh, these data require a cross-cutting policy: early detection, school and student health, occupational medicine, support for farmers and sailors, prevention of isolation, rapid access to psychiatry, and continuity after an attempt.

5.2 A renewed sector psychiatry

Following our contacts, it has become clear that psychiatry must become accessible again before the crisis. Each community must have teams on the ground, medical-psychological centers with measured timelines, solutions for children and adolescents, mobile teams, and beds for acute situations. Professional, school, hospital, and social workers must share simple referral systems with the patient's consent.

An accessible 24/7 response for guidance and crisis will articulate health regulation, mobile teams and appointment-free reception centers. Joint mental health and emergency teams will be able to intervene when the alternative would be an exclusively police response, with strict protocols of safety, consent and evaluation.

5.3 Consolidate the monitoring systems

The 3114 and VigilanS4 They provide a capacity for response and follow-up. Their effectiveness depends on the ability to actually direct patients towards care. Indicators should therefore track the waiting time until the first consultation and continuity of the care pathway, not just the number of calls or registrations.

6. Prevention, the environment and inequalities

6.1 Rebalancing prevention and care

For Yes Breizh, prevention in Brittany remains scattered and poorly organized. It should be organized around objectives by departments (or countries): vaccination, screening, physical activity, oral health, nutrition, sexual health, perinatal care, occupational risks, and healthy aging. The actions must be evaluated and targeted towards the populations that benefit least from them.

Preventive and diagnostic appointments should be offered in regular and identified locations – pharmacies, health centers, municipalities, workplaces, and sports associations – for cardiovascular, respiratory, and certain cancer risks. These should lead to a real process of diagnostic confirmation and treatment, so as not to create screening without follow-up.

6.2 Alcohol, tobacco and other addictions

For Yes Breizh, risky consumption is neither a matter of morality nor of cultural identity. It must be addressed through information, risk reduction, early detection, care, and action in commercial and social environments. The 2024 Baromètre de Santé publique France now provides regional data that will allow for monitoring of developments.

6.3 One health: human, animal and environmental

Water and air quality, pesticides, green algae, occupational exposures, habitat, noise, zoonoses, and climate change have health consequences. A «One Health» approach must combine human health, animal health, and ecosystems, with open data, independent research, and coordinated decision-making.

6.4 Reducing social inequalities in health

Forgoing care depends on income, transportation, housing, and out-of-pocket costs. Average figures may mask significant differences. Each program must publish its results according to the department (or country) and social characteristics, while maintaining confidentiality.

6.5 Building a Breton strategy for women's health

Yes Breizh concluded from its reflections that women's health care pathways remain insufficiently coordinated between prevention, general medicine, gynecology, maternity, mental health and research. A Breton strategy must be developed to organize access to contraception and abortion, prevention and screening, menstrual health, endometriosis, menopause, perinatal care and perinatal mental health. An independent audit of maternity hospitals must be able to simultaneously examine safety, territorial accessibility, staffing and the experience of women and families.

7. Professionals: training, retention and protection

7.1 Plan the training needs

For Yes Breizh, training capacities must be linked to ten-year needs: doctors, nurses, midwives, nursing assistants, pharmacists, radiographers, psychologists, psychiatrists, dentists, and home-care workers. The professional-defined internships must be distributed across all life zones and accompanied by housing, transportation, and recognized internship supervisors.

7.2 Free up time for caregiving

Shared secretariats, medical assistants, cooperation protocols and interoperable digital systems can reduce tasks that are of no clinical value. The transfer of skills must be trained, protocolled and evaluated; it must not become a mere transfer of workload.

7.3 Improving working conditions

For Yes Breizh, loyalty comes down to staff, planning, remuneration, management, the prevention of burnout and access to housing. The health of caregivers is an indicator of the health of the system. Institutions receiving public funding will publish comparable social indicators.

7.4 Recognizing caregivers and consolidating palliative care

According to Yes Breizh, caregivers contribute to the continuity of life, but must not become the adjustment variable of the system. An effective right to respite, an annual assessment of their health, and simple information about their rights will be organized by living area. Palliative care, including pediatric care, must be accessible at home, in institutions, and in hospitals, with trained teams and a multi-year funding plan that is transparent.

8. Social protection and financial sustainability

8.1 Preserving solidarity

In 2025, the total deficit of the French mandatory basic schemes and the Old-Age Solidarity Fund reached 21.6 billion euros (in proportion for Brittany, 1.57 billion), of which 15.9 billion for the sickness branch. The LFSS predicted a deficit of 19.4 billion in 2026 (in proportion for Brittany, 1.41 billion), with execution risks highlighted by the Court of Accounts.

These figures impose a credible trajectory, but a health budget cannot be managed like a company's budget. For Yes Breizh, useful spending sometimes avoids future costs and protects social cohesion.

For Yes Breizh, the good question is twofold: what stable resources does society devote to health and what health value does it obtain for each euro?

8.2 Holding all actors accountable

The relevance concerns prescribers, institutions, industry, transport, complementary healthcare providers and patients. The fight against fraud and errors must be strengthened without creating obstacles for legitimate policyholders. Yes Breizh wants to see the purchasing of medicines and devices become more transparent, more easily evaluated and with greater negotiating power.

8.3 A Breton envelope with a guarantee

In a devolution framework, Brittany would receive a multi-year envelope adjusted to age, precariousness, rurality, tourist flows, and health needs. Justice would ensure the equalization and coverage of heavy risks. The savings obtained through prevention and organization could be reinvested in the Breton system, rather than automatically being taken back.

9. What the devolution would change

9.1 A Brittany health authority covering five departments

This authority would integrate the territorial functions currently scattered between two ARS, in cooperation with the Assurance Maladie, the Départements, the communes and the professionals. Its director or directorate would be accountable to the Breton institutions after a public hearing, and would therefore not be appointed solely by the French central government.

9.2 A Brittany Assembly setting the priorities

The Assembly would vote on a health strategy and a multi-year budget, after public debate. It would not decide on medical treatments: clinical recommendations would remain based on science and developed by the competent authorities. It would control access, organization, results, and the use of the means.

9.3 Local councils with the necessary resources

The Living Quarters would draft enforceable contracts specifying services, deadlines, staffing levels, and mobility solutions. Professionals, patients, caregivers, and elected officials would have a genuine power of alert and proposal. Any major reorganization would publish a territorial and health impact study. The 2026 French national barometer shows the extent of local expectations: 70 % % of all French people attribute a role to their municipality in matters of health, but % only 42 believe that it is actually acting. The devolution must enable, in Brittany, to transform this expectation into responsibility accompanied by competencies, resources, and an obligation to deliver results.

9.4 Non-negotiable national rights

The devolution would respect a principle of non-regression: the same rights to reimbursement and social protection, the patient's free choice, continuity during a move and equal access to innovations. The French national authorities would retain the sovereign functions of health safety, drug authorization and crisis management that today extend beyond Brittany because they often fall within a European level.

10. Sixteen proposals for a Brittany that is healthier

Proposal 1 – Create the Breton Observatory5 from health to five departments

It will bring together demographic, environmental, social, care and outcome data. The indicators will be published by living area: actual timeframes, patients without a primary care physician, forgoing care, avoidable mortality, mental health, air and water quality, and available staff and beds.

Proposal 2 – Guaranteeing a primary care team in every living area

Each local living area (department or country) will have a contract combining doctors, nurses, pharmacists, midwives and other professionals. The model can be liberal, salaried or mixed. A solidarity mechanism will sustainably fund areas where activity alone does not allow for balance.

Proposal 3 – Deploy a Breton plan for internships and setting up in practice

All health students will have access to internships located locally with accommodation and transportation facilities. Internship supervisors will be trained and compensated. Support for studies in exchange for the commitment to practice will be supervised, proportionate, and prioritized when they respond to a collective territorial project.

Proposition 4 – Establishing the Alliance Hospitalière de Bretagne

The CHU hospitals of Nantes, Rennes and Brest will coordinate rare care, research, training and procurement with the general and nearby hospitals. A graduated map will specify the accessible services, transfers and target times. No closure will precede the implementation of a secure solution.

Proposal 5 – Organize a care center by life zone

Medical outpatient clinics, access to healthcare services, pharmacies, medical transport, nursing care and mobile teams will be planned together. Emergency departments will have back-up beds and geriatric and psychiatric solutions.

Proposition 6 – Making mental health the main priority for Brittany in 2027-2032

The plan will set target deadlines for medical-psychological centers, strengthen pediatric psychiatry, student health, mobile teams, prevention in the workplace, and follow-up after a suicide attempt. Each school and university will have a known orientation path.

Proposal 7 – Allocate a growing share of the budget to prevention

A multi-year, protected envelope will fund programs evaluated on vaccination, screening, addictions, physical activity, oral health, perinatal care, and aging. Funding will follow the inequalities in needs, not the local capacity alone to develop dossiers.

Proposition 8 – Create a «One Health Brittany» program»

It will cross-reference human, animal and environmental data for water, air, pesticides, zoonoses, occupational exposures and the climate. An independent scientific committee will publish uncertainties, conflicts of interest and recommendations.

Proposition 9 – Guaranteeing access to healthcare in the islands and locally

Mobile teams, advanced consultations, transportation, temporary accommodation and telemedicine will reduce the loss of opportunity. Indicators will incorporate travel times and seasonal variations.

Proposal 10 – Create an interoperable route file

With the patient's consent and in accordance with the law, authorized professionals will access the useful information. The goal is to avoid repetitions, information losses, and unnecessary examinations. Any secondary use of the data will be subject to public governance and traceability.

Proposal 11 – Establish an organizational innovation fund

The fund will support employee-run health centers, intercommunal collaborations, mobile teams, and new task sharing. The experiments will be limited in time, compared and generalized only if they improve quality, access, and working conditions.

Proposal 12 – Establish an annual health conference

Patients, caregivers, professionals and elected officials will review the results and priorities. The Breton Authority will have to publicly respond to the recommendations and justify the discrepancies between objectives, budgets and achievements.

Proposition 13 – Implementing a Breton plan for hospital and home care fluidity

A cell organized by the local unit will monitor beds, delayed discharges, transportation, and the capabilities of the home and the social-medical team. Home hospitalization will be offered on medical recommendation, with consent, twenty-four-hour continuity, and explicit support from caregivers.

Proposal 14 – Opening of nearby prevention and diagnostic centers

Coordinated appointments will be offered at health centers, pharmacies, and other accessible locations for cardiovascular and respiratory risks as well as certain cancers. Any positive screening result will lead to a planned referral and a target timeframe for confirmation.

Proposal 15 – Adopt a Breton strategy for women's health

It will cover menstrual health, endometriosis, contraception, abortion, menopause, perinatal care, perinatal mental health, and research. An audit of maternity hospitals will jointly evaluate safety, staffing, access times, and women's experience before any reorganization.

Proposal 16 – Guaranteeing respite for caregivers and access to palliative care

Each caregiver will be able to benefit from a health assessment, coordinated information and an adapted respite solution. A Breton palliative care program will determine the necessary capacities at home, in institutions and in hospitals, including for children.

Conditions of implementation

No transformation is generalized without a budget, a market opportunity, revenue and evaluation. No objective is imposed without a budget, a timeline, support, and accessible technical solution. No transfer takes place without resources. In the event of failure, the system is corrected rather than maintained for appearances’ sake. A code green, orange, red It will indicate the trajectory, but will never replace the figures and their explanation. A temporary increase in an indicator may correspond to a transition or to a better census: each evolution will be interpreted before a decision is made.

11. Average figures must never mask a loss of territorial opportunity.

Conclusion – Health as evidence of devolution

According to Yes Breizh, health is the area where devolution would be judged most directly: an appointment obtained, an emergency taken care of, a professional retained, a suicide avoided, water better protected. It cannot be achieved through a simple change in organizational structure.

Brittany must retain the forces of national solidarity and French medicine (including Breton medicine), while gaining the ability to organize its responses. The proposed framework combines central rights, Breton responsibility, and territorial implementation. It gives professionals more control over their organization and Breton women and men greater control over the results.

Yes Breizh's principles for health: prevent before repairing, organize before closing, cooperate before centralizing. An autonomous Brittany in health would not promise that everything would be available everywhere; it would guarantee that no one would be left behind because of their address, income, or age.

Main sources

Court of Auditors, Social Security – Report on the implementation of the laws on the financing of social security, Synthesis, May 2026.

Document submitted, «Health in Brittany», local experiences related to access to healthcare, 2026.

French Hospital Federation, Verian Institute and Jean-Jaurès Foundation, Observatory of Territories, «health» wave of the municipal barometer, January 21, 2026 (national survey of 1,000 adults).

Breizh Europa, Health and social protection, status report published in 2019.

ARS Bretagne, Regional Health Project 2023-2028 and territorial diagnosis.

Public Health France, Baromètre 2024 – Bretagne edition, published in April 2026.

Public Health France, Suicide-related behavior in Brittany – 2024 report, October 2025.

Insee, demographic balance of Brittany 2025, February 2026.

DREES, work on accessibility for healthcare professionals and the health status of the population.

Health Insurance, Observatory on Access to Healthcare and Medical Agreement 2024-2029.

Scottish National Party, On Scotland's Side – Government Programme 2026, May 2026 (close-to-home care, hospital fluidity, mental health, women's health, caregivers and palliative care).

Notes and clarifications

1. By principle, Yes Breizh avoids the term ‘territoire’ in its publications.
